Use this form to authorize Canyon Lakes Property Owners Association (hereafter referred to as “the Association”) to send you notices, mailings, invoices, and statements via electronic mail. Email addresses and phone numbers obtained from property homeowners will be used only for the purposes identified in this agreement and will not be disclosed to a third party. Please complete ALL information indicated with an asterisk (*) below. When you click submit, the form will be emailed to the Association office.

SECTION 1: Electronic Mail Consent

By checking this box I, the undersigned, hereby agree to opt in to receiving electronic mail from the Association and that the terms “mailed” and/or “delivered” for the purpose of giving or service of any notice, or any other document, required or permitted by The Revised Code of Washington or by the Association’s Master Declaration of Covenants, Conditions, and Restrictions; and/or By-laws; and/or rules, is deemed effective notice to such property owner(s) if and at the time such notice is transmitted by electronic means to such email address(es) as recorded below. I further understand that it is my responsibility to notify the Association in writing of email address changes and that I can revoke my consent to electronic mail delivery by written notice delivered to the Association.

SECTION 2: E-Signature

Electronic Signature Agreement
